Q. What to do with six feet tall fruit bearing apple tree in good soil that leans about 45 degrees with or without fruit

The tree may be leaning towards the sun. If it is a fairly young tree it can be dug up and repositioned at a slight angle away from the maximum exposure to the sun. It will grow towards the sun which should balance the overall shape of the tree.
