What do you do about orchid roots or stems coming above soil?
Do you cut them, replant in larger container and add soil? Or, do you just leave them hanging?

Dear Orchid Grower: When the roots have become too numerous, it is time to repot. During repotting, cut off any roots that are dead. Increase the size of the container so that you can gently put the new roots into the new pot. I do leave them hanging until it is time to repot (when the orchid medium is old or when there are many roots and they need to be contained in the container)
